There is one available at the Isolated Merchant at the Weeping Peninsula for 3000 runes. At the very west of the peninsula, you can find his shack.

Isolated Merchant at Raya Lucaria sells them for 4000 runes. After passing through the blue seal on the Main Academy Gate, walk down the hill to the east, then to the south.

Liurnia Of The Lakes

A water wheel lift can be found at the bottom of the cliff near Raya Lucaria Academy’s Schoolhouse Classroom Site of Grace. There will be one here (After riding the lift down, jump up the crystals to the west.)

The Hermit Merchant at Ainsel River sells one for 3000 runes.

The Isolated Merchant at Dragonbarrow sells two for 5,000 runes each.

There will be a large cavern with giant purple crystals on Sellia Highway. It will be on a corpse. There will be a Revenant guarding the lost ashes of war, so be cautious.

In the Sage’s Cave in the Altus Plateau, look in the first chest past the Skeletal warriors.

In Leyndell sewer tunnels, past minor enemies, you will find the lost ash at the end of a long tunnel. Be cautious, as a Fingercreeper is clinging onto the wall guarding it.

Outside Dung Eater’s cell, there will be one on the hanging corpse. As you approach, a gigantic hand monster will pop out of the ground.

Subterranean Shunning-Grounds

On the narrow ridge in the Mountaintops of The Giants there will be a corpse with an item shortly after the enemy with the whip.

The Imprisoned Merchant inside the Mohgwyn Dynasty Mausoleum sells one for 5,000 runes.

In Miquella’s Haligtree canopy, there will be a corpse on a statue; you may find the final one in this location.

How To Use Lost Ashes Of War

Lost Ashes of War have no affinity by themselves, and you can’t use them without bringing them to a smith. You can travel to the Roundtable and present Smithing Master Hewg with your Lost Ashes of War. In return, he will allow you to duplicate a normal Ash of War in your inventory.
